hi! my 18mo old has diaper rash which did not clear with the use of nystacid and clotrimazole ointments, i noticed a prominent circle formation and tried my sons repivate ointment and it cleared it in 2 days, is ringworm possible on the diaper area?

Repivate and diaper : Repivate is  a  steroid  Steroids as  anti-inflammatories   stop iimmune response  and the rash of diaper rash  the immune response tot an infection ( fungus) . So the steroid is masking the rash but not treating the underlying cause which is fungus. Stop the repivate,  consider  desitin a zinc ointment that  is a barrier  well as fights fungus and f/u /w /doc this week.

Redness, peeling..: Moisture, heat and lack of air circulation may cause skin irritation which may get over infected with bacteria or fungus. Redness and peeling may indicate exposure to moisture, blisters may indicate a bacterial infection and if associated with satellite lesions, a fungal infection. It's better to see a doctor for a definitive diagnosis.

Clean, dry skin: Diaper rash can be best managed with very absorbent diapers that are not placed tightly and get changed soon after soiling. Hygiene is the most important way to avoid this rash. Do not use alcohol/perfumed wipes as they may overly dry the skin and irritate it. Allow your baby to air dry, the longer they can go without a diaper the better. Over the counter pure white petrolatum ointment may help.

Diaper rash care: He might need a cream from his pediatrician such as Nystatin but in the mean time , there are some things you can do.  You need to put a good barrier cream on the diaper area every diaper change such as triple paste. Stop using baby wipes for now and just use a soft paper towel and water.  Change the diaper more frequently.  If possible, airing out the diaper area will help as well.

Desitin (zinc oxide diaper cream): Desitin (zinc oxide diaper cream) (no financial interest) has been my favorite for many years, along with allowing the baby to crawl around without a diaper as often as is practical. (you can't have diaper rash with no diaper!)  if the rash does not resolve, or worsens, then something else (like a yeast or fungus infection) may be going on, and you'll want to see your pediatrician or a dermatologist.

Stay dry: Keep the area as dry as possible.  If there is itching & redness, some over the counter medications may help (antifungals).  Be careful using otc steroid creams.  If diabetic, see your pcp.  Wear cotton underwear or none at all when possible but if wearing diapers, change ofen, wash often, dry often, use absorbent material like corn starch and use barrier creams (some have zinc).  If ?'s, see pcp!
