Depends: There are many causes of elevated Red cell count and hemoglobin and it is good that your brother is seeing a doctor for a proper diagnosis. It is not feasible to provide a meaningful opinion without examination and some tests.

Not very significant: Slight increase in RBC's may be a  compensatory reaction to blood loss either external from injury or internal like a bleeding  ulcer. Excessive increase in RBC count can be due to bone marrow disease. Repeat the count after a few months and if it is still high consult a hematologist

5 months post covid 19 my red blood cell count is marginally high (5.69 vs range of 4.5-5.5) which may explain why i feel like i am not getting enough oxygen. other blood tests look fine but asthma and allergy meds are not helping. ideas welcome.

Symptoms: This red blood count would not lead to a shortage of oxygen.  There are long term effects of covid so breathing difficulties from covid lung damage, for instance , may persist.  However, being sick with this illness can be very scary.  It is possible that anxiety about breathing is leading to problems.

: As a consequence of a positive skin punch biopsy, for small fiber neuropathy, it is next appropriate to obtain blood and urine studies to uncover causation.  The most common cause is diabetes or pre-diabetes.  Low B-12 may be a cause but low folate is NOT an etiology.
