Dialysis patients can not regulate potassium and salt very well, so they need to minimize these minerals in their diets.  If they have too much dietary salt and potassium, cardic issues and and an elevated bood pressure can occur.

The kidneys normally excrete potassium, so patients with renal failure will have a tendency for their potassium levels to rise.  The dialysis machine removes excess potassium from the body, just like the kidneys do.  Taking a diet low in potassium is helpful in keeping potassium levels controlled.

Difficult balance: You describe one of the most complicated scenarios in medicine: a person with clinically signification heart failure, lung disease and now renal failure. Not everyone with chronic kidney disease needs dialysis. If his heart failure medications and diuretics can be adjusted he may be able to avoid dialysis for a long time.  If he has acute kidney failure.. The options are less.

Sometimes: High potassium foods can continue the potassium problem but high potassium means that the kidneys are not handling the potassium load. Hypertension meds such as ACE inhibitors ("prils") and potassium sparing diuretics can inhibit potassium excretion by the kidneys. Kidney disease along with high blood acid levels can have the same problem. Check with your doctor and perhaps a nephrologist for why!
