Sinus infection: Migraine is not associated with fever but what you have sounds like a severe sinus infection. You need to see your doctor now.

Circumvallate papill: You have just discovered one of your taste buds: called the circumvallate  papillae.  There are about 9 of them back there on the dorsum (top of) the tongue.  Check the anatomy pictures.  They are arranged in an upside v fashion.  Good luck.  Congrats !

do i  have tb risk? iam from the a country where tb is common. i have thick white mucus/phlegm, but no cough. how ever there's a time that i spit blood with it (twice/thrice only) today i spit with yellow mucous i done a chest x ray and its negative.

TB test: I would suggest an evaluation and upper airway examination. A TB test is advised.

Noisy breather: Babies breath through their noses an get a lot of mucus in the nose. Use a bulb syringe and suck out the mucus. Use salt water nose drops  4times daily.Use a vaporizer and elevate her mattress.

COVID concerns: COVID 19 Sx's may include fever, cough, feeling tired, shortness of breath, muscle aches, headache, sore throat, chills, chest pain, nasal congestion, loss of taste or smell, nausea, vomiting, diarrhea. Your symptoms are not particularly suspicious for COVID 19. Monitor for other symptoms.

Phlegm: If you are referring to post nasal drip then i would recommend trying a nasal spray- steroid/ anticholinergic/ antihistimine. Talk to your doctor to see which is best for you!

Common: COVID-19 can have no symptoms at all, minor symptoms lasting a few day, more severe symptoms lasting several weeks or, occasionally, severe symptoms lasting several months.  No way to predict, treatment is very uncertain.

Check in with your doctor to make sure pneumonia is not present

Cancer? See doc: I don't understand how you produce phlegm without coughing. Is this post-nasal drip which you spit out? In any case, that's a worrisome symptom, and cancer is on the list of possibilities. You should see a physician promptly.
