Careful history: There are many kinds of illnesses in which a person can experience psychosis. Schizophrenia is one such illness - probably one of the most severe illness of this kind. Careful observation and interviewing of the patient and of patient's loved ones is usually what helps the psychiatrist to make a particular diagnosis. Hope this helps.

Not so clearly : Functional brain scans are being researched, but are not yet reliable enough to be a clinical tool. Talking with the person is much more helpful; seeing what s/he has to say & how it's said. How language is; thoughts expressed; what is the emotional tone or lack of it.  Is s/he oriented to person, place, time, or not?  Are there neurological signs that need further examination, etc. Human contact.

No limit: I am not sure what you mean by this question. Persons who suffer from schizophrenia may suffer from chronic, ongoing delusons or may suffer from periods of exacerbation of their illness. There is no limit to the number of episodes of decompensation in a person's lifetime. Many persons may suffer from waxing and waning illness with variable impairment in level of functioning.

Excellent question!: ...But the list of things that can result in psychosis is long and complex so- it could be something as simple as sleep deprivation or something as sporadic as complex-partial seizure disorder. The most common response to your question would be "a chemical imbalance in the frontal lobes of your brain, such as too much dopamine"... But this is rarely actually proven! read "running from the devil".

Sleep deprivation : Sleep hygiene is extremely important for everyone but particularly for patients with a mental illness - prolonged sleep deprivation can trigger manic symptoms , lead to confusion.  Disorientation and yes could exacerbate psychosis.

Don't know, but...: Without having professionally evaluated a person it's impossible to say what underlies their behaviors. Your psychiatrist or therapist can best answer your question. A persistent (due to personality) / transient (e.g., d/t psychosis or mania) tendency to blame others combined with reduced impulse control (e.g., d/t psychosis/ mania/ intoxication) could be one way a good person cud become violent.

would a person that was always agitated / angry change after taking abilify (aripiprazole)? possible schizophrenia / first time psychosis

Depends: The answer depends on the underlying cause of these symptoms.  Abilify (aripiprazole) would tend to decrease agitation and anger and it is one of the treatments of choice for psychosis.
