Oral antifungals: If topical therapy is not helping, the next step in my opinion is a course of oral (systemic) antifungal medication. There are different regiments, but you should talk with your PCP about the pros/cons of each one. These medications can be hard on the liver and oftentimes must be taken for several weeks.

Maybe, but...: It's possible that it does, at least according to the press release that got so much attention.  Then again, lots of things kill hiv...Soap and water, for example.  What we need is something that extracts HIV dna from human dna.  I doubt that this toenail cream can do that!

Ciclopirox: A topical antifungal.  It may cause dry skin, allergic reactions, but no serious systemic side-effects have been noted as a result of topical application.

It depends: Ciclopirox cream is used to treat infections caused by fungus, and therefore may be used to treat ringworm, candida, tinea versicolor and possibly seborrheic dermatitis. So, while it would likely be safe to use this cream if may not be helpful. See a dermatologist to determine the possibly cause of your condition. Once a diagnosis is made then a proper treatment can be prescribed.
