Testicle Pain: Testicle discomfort usually comes from inflammation, not infection. Antibiotics are useful, but alone don't always help.
I suspect you have prostatitis as the cause.  Prostatitis can refer pain to the testicles, penis, groin and more. 
Increase water intake, reduce caffeine, begin nsaids (aleve) with food twice a day for two weeks. Hot soaks in a tub helps. F/u with md if not better in 7-10 days.

No: The spermatocele is, by definition, a cyst arising from the epididymis which is directly attached to the testicle.

Masturbation/Numbnes: If your sensation is normal later in the day, its likely due to your technique and how much the tip is handled.  If it remains with decreased sensation, you should see your local doctor.

Male exam: certainly get examined and checked for infection and hernia. You will need a full examination to rule out a penis, urinary or testicle infection and evaluation for lymph node swelling. If unprotected sexually get an STD screen.
