However, if your are sexual active and you do not use protection, genital warts due to HPV infection could be the case. You should see a dermatologist if possible or at least your family doctor and use condom in the mean time.

Scrotum infection : You have an infection that mupiracin won’t help, because infection is inside and mupiracin , as a topical antibiotic, only affects certain superficial skin bacteria .
    You need to be seen by a doctor. You probably need systemic antibiotics and possibly a drainage procedure.

Itching: Most common cause of itching in genital area, groin and buttocks for males is jock itch, which is a type of fungal infection, also called ring worm, although it is really not a worm. It is easily diagnosed by exam by your doc who can also do skin scrapping if doubtful for treatment with OTC meds.

Penile pain: It sounds like you have already had an extensive negative (normal results) work up. However the description of your pain sounds like it could be neurological origin. Perhaps check with a neurologist to assess possible sacral nerve dysfunction.
