Plain blood from urinary opening can be due to urethritis, bladder stone, urethral polyps, hpv, std or prostate cancer. You should see a urologist for diagnosis ; treatment.

Sometimes this will produce small tears in the frenum, or attachment of the foreskin.That should be visible to  close examination (mirror might help) .If this comes out of your pee hole, it is unexpected and should be evaluated by a urologist.
