Bleeding after sex: Bleeding and pain after intercourse is abnormal and needs investigation.The causes can range from cervical polyp and ovarian cysts to cervical cancer , pelvic inflammatory disease and endometriosis.At the least, you should have a pelvic exam, pap smear, a transvaginal sonogram, a pregnancy test to rule out ectopic pregnancy.In a chronic condition, consider laparoscopy.

?Endometriosis: Please see your OB/GYN or primary doctor and have a frank discussion with them about how often, when, what positions, etc. cause this severe pain.  This is NOT normal, and can be a sign of more serious illness.  Endometriosis (inner lining of the uterus escaped to OUTSIDE the uterus, but inside the belly) can sometimes cause scar tissue which is stretched with sex and orgasm.  Get care!  Good luck

Dysorgasmia: Dysorgasmia is the term used to describe pain with orgasm. Likely more common than suspected in women. Unfortunately, there is a lack of good, validated, scientific evidence on causes and treatment. Seems to be more common in pre-menopausal women. Suggested causes include hormone imbalance and pelvic muscle spasm. Treatment options: NSAIDs, heating pad, hormones, pelvic floor PT, amitriptyline.

Pudendal Neuralgia: Sounds like a pudendal neuralgia. The pudendal nerve provides sensation and function to the genitals.  If it is irritated or entrapped in any way it can hurt during sex.  You make consider seeing a women's pelvic pain specialist for pelvic floor treatment and therapy.

PAIN: Could be an ejaculatory duct obstruction. A transrectal ultrasound is the study of choice and will also see the prostate in detail.
