Menstrual pain: Ultrasoudn woudl find polyps, fibroids and ovarian cysts but may not see  endometriosis which  can be diagnosed on laproscopy. Would discuss the possibility with your OB

Need to examine: It is not feasible to provide a meaningful opinion without taking additional history, physical examination and may be some tests. It would be prudent to see your doctor. 
Your symptoms suggest pelvic inflammatory disease and you need to be evaluated by an in person examination.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex. 
Get HPV vaccine

Unrelated: Antibiotics and steroids would not be prescribed for an irregular menstrual cycle. If they were prescribed for something else by a doctor then the decision to use them would not be altered based on the status of the menstrual cycle or based on a miscarriage a few months ago

Heavy periods.: There are multiple options for heavy periods. Hormonal treatments such as birth control pills, Progesterone treatments and the Mirena (levonorgestrel) iud are effective reversible treatments that will help many people. There is also a drug called Lysteda that contains no hormones which will help others. Vaginal odor is usually bacterial vaginosis but confirmation will be required.

Uterine bleeding ?'s: Passing clots following after a menstrual period may indicate pregnancy outside the uterus (ectopic). See your gyn physician for an exam and pelvic ultrasound, and obtain a serum HCG to be sure about pregnancy.

See below: This usually means there is very heavy flow of blood duing your menstrual period.  A variety of things can cause this incding small smooth muscle tumors of the uterus which protrude into the endometrial cavity.  Older women also get this becaue of irregualr period intervals  these are the basic casues.

The way you are: If you are talking about seeing blood clots coming out of your vagina, that can be normal.  Just like if you cut your skin and then it clots and forms a scab, the blood coming out of your uterus can clot inside of your vagina before it comes out, especially if you are lying down or sitting down for more than about an hour.
