Varicocele: Might be varicocele do you feel a "bag of worms in your left testis other options are hernia, infection, or if previous vasectomy, post vasectomy syndrome.

Prostate Source: Testicle discomfort usually comes from inflammation, not infection. Antibiotics are useful, but alone don't always help.
I suspect you have prostatitis as the cause.  Prostatitis can refer pain to the testicles, penis, groin and more. 
Increase water intake, reduce caffeine, begin nsaids (aleve) with food twice a day for two weeks. Hot soaks in a tub helps. F/u with md if not better in 7-10 days.

Maybe the ER: If it is sudden severe testicular pain you have to go to the emergency room. It might be a testicular torsion, meaning the testicle is twisted and is not getting enough blood supply. This is an emergency because the testicle might die from this.  You would need quick treatment.

Get evaluated.: There are many causes of testicular pain and you should see a physician for a history, physical examination, and then possibly a scrotal ultrasound. The most worrisome is torsion, when the blood supply to the testicle is twisted closed. Infections can result in epididymitis or orchitis.  Others causes are more benign, such as hydroceles & spermatoceles. A full workup will help determine the cause.

every once in a while i get a mild pain on my left testicle. i don't notice any swelling of any sort. what could this be?

Multiple things: The differential diagnosis for testicular pain is very broad and can include pain referred from the groin from hernias or structural problems with the spermatic cord.  Immediate pain that does not subside is an emergency.  Even without swelling, a serious cause is a possibility, and testicular pain warrants an examination by a urologist, basic urine and blood tests, and a sonogram of the testicle.

Testicular pain: I understand that it may be awkward, but if you are having testicular pain, you should see your family doctor. There are several causes of testicular pain and some of them are serious. It is better to find out if there is a serious problem earlier on to have the right treatment.

i had gono and was treated with 500mg ceftriaxone iv, 3 weeks ago. all symptoms gone except intermittent small pain on my left testicle. shld i worry?

Gonorrhea: 500 mg of Ceftriaxone i/v seems like a little too excessive vs. 125 mg i/m as recommended in the us. The pain in the testicle may be residual, but if not gone away within the next few days, you will need to go see doc. One way or the other, i recommend you be retested to make sure gonorrhea has been cured.

Testicular pain: A full exam and some test can better determine if it is anything serious. If there is no blood in urine or ejaculate or mass on testicle than likely nerve pain that is fleeting. Need to check for infection and have an exam for hernias varicoceles hydroceles masses or lymph nodes.
