Eczema: I suspect the most common cause of this is simple eczema, but some skin cancers can behave this way as well.  I saw a young woman recently who simply had dirt build up in the ear canal that looked just like this.  So the best answer is to have your ear looked at by a doctor, who can usually tell with nothing more than simply taking a look with their trained eyes as to what is the next step for you.

Wax or...: Typically occulsion of the ear canal by excessive wax or significant serous fluid in the middle ear can cause the abrupt onset of these symptoms.  A simple exam by your doctor with an otoscope will provide the answer.

Ear: A common cause is tensor tympani myoclonus where a the tendon attached to one of your small ear bones may go into spasm especially when in noise. See an ENT to check it out. You will probably need a hearing test

Doctor can evaluate: An ear infection on the inside of the eardrum is a "middle ear" infection, and can start when a person gets congestion from allergies, colds, or sinusitis.  An ear infection on the outside of the eardrum is an "outer ear" infection or "swimmer's ear", which is caused by moisture in the outer ear canal that allows skin germs to grow and multiply in the skin. Other causes of pain are possible.

Throat: Get a strep throat test, have doctor look at ear (rule out ear infection). For now take hall lozenges , fluids, tylenol (acetaminophen) or motrin. Decongestant if congested.

Could be either or: it could be the ear ache caused by infection that started the circle and end up with bad headache.
or it could be a sinus headache started and spread to the ear and caused pain as well.
stop the circle by treating one and the other will follow.
