?hydrocele or testis: Larger side might have a hydrocele, tumor or cyst. Smaller testicle might have been previously  corrected undescended testis, munps in past or presence of a large varicocele. Testes can grow at different rates during puberty ; smaller one may or may not catch up in time ; both testes can otherwise be normal. Thus although not normal maybe ok. Scrotal ultrasound would clarify.

Not normal: But need not be a problem unless smaller testis is much higher than the other, have had mumps in the past, or have a large varicocele or hernia on small side. Thus although not normal maybe ok. Scrotal ultrasound would clarify.

See doc now: If the enlargement of your testicle is new and has not been examined by a doctor, it needs to be examined right away. There are benign reasons for masses in the scrotum, but testicular cancer is always a concern. Go get it examined now.

Anatomy: Yes. Of course. One testicle is always more dominant than another.

Large painful testis: If it 'transilluminates' it could be a hydroceole.  Epididymo-orchitis can also be painful and cause swelling.  A hernia into the scrotum is another possibility.

You should see a urologist.

Doctor can examine: His pediatrician can take a look to decide what to do. Testes don't have to be matching in size exactly, but if one is a lot bigger than the other, then something may be wrong with either the one that is too huge or the one that is too tiny. If a 2 year-old's testis is an egg-shaped thing that is sometimes bigger and sometimes smaller, it may have fluid around it, which is treated by a surgery.

Testicle Anatomy: It sound pretty normal to me.  One testicle is the dominant one and one is the backup in case of injury to the dominant one.  Healthy testicles are almond shaped.

Clinically, bring the info on the onset, degree, duration, interval, evolution, progress, &amp; resolution of the pain &amp; swelling and their potentially related Sx to urologist for analysis + exam + tests so to ensure NO recurrent testicular torsion with spontaneous detorsion or STD-related urethritis leading to epididymitis or hydrocele...

No: If this has indeed happened, you should be examined right away. All adult makes should be performing routine monthly self testicular exams.
