Need evaluation: This really can be anything.  Depending on age, history, clinical scenario (e.g. Recent surgery), etc..., a variety of diagnoses can be applied.  Need more information that only a thorough history & physical can provide.

There : There is always a possibility of pregnancy if you are sexually active. Have you missed a period yet? As doctors, the first thing we do when we hear that a young, fertile woman is having a missed period or abnormal spotting, we check a pregnancy test. You can either take a home pregnancy test as long as it has been 5- 6 weeks since your last normal period, or you can go see your doctor and they can test you. A home pregnancy test is 97% accurate if taken a week or more after the missed period. A blood test can tell if you are pregnant as soon as 7 days after conception and a doctor's urine pregnancy test can tell if you are pregnant around when you next period should come.  Since your abdominal pains have been going on for a year, they are likely not from pregnancy so you should give your doctor a call anyway for an evaluation.  Good luck!

Possible nerve pinch: The sensation of cold can occur do to a pinched nerve in the back or other Orthopaedic problem. I would advise you see you PCP for further evaluation.

Possibilities: Although there are many problems that  could be causing your symptoms, it is fairly common pattern that people with ulcer disease will have their most relief when they are eating, as the food tends to buffer or protect the ulcerated area. No one can truly know what is causing your symptoms without investigation.

Muscular?: Interestingly, this could very well be a muscular pain. We don't think of muscles in that area, but pain in the psoas muscle can refer there. Granted, you haven't described any digestive-type symptoms so I'm assuming you're not having any. And inflammation of an organ or part of the intestine sitting atop the psoas muscle can cause a similar issue.
