Too complex for here: Many details will influence the diagnostic pursuit.  T5 thoracic a little high to produce abdominal pain, but due to some anatomical features might be possible.  "Severe pain attack" sounds like single episode.  Were there more?  If actual neuralgia ("neuritis") then pain pills won't work.  But, best to get diagnosis.  Chasing symptoms will probably not work well. Need MR of thoracic spine.

Not sure of what context this was introduced, but perhaps this refers to pain radiating from your spine down a leg.  If occurring after back surgery, perhaps post-op scarring or arachnoiditis.

Injection: Although the pain of greater occipital neuritis can respond to analgesic meds, I have found that localized nerve blocks, even local cortisol may reverse and stabilize indefinitely.  A permanent solution may be associated with an alcohol injection, but this will leave some numbness over the ear.
