Pinched Nerve: Sounds like you have a pinched nerve in your back.  I would suggest seeing a pain/spine specialist to evaluate you further to evaluate what level and what can be done to treat you.  There are interventional treatments besides medications and surgeries that might reduce or  eliminate the pain altogether.

Sharp breast pain: may be early shingles infection,  may be from bra irritation ( check size especially if gained weight), reflux (try ranitidine/prevecid),   check TSH, CXR,  You also need mammogram just to be sure not a mass/lesion.

Not very: These are done with local anesthesia and with very thin needles.  After the anesthesia wears off you will be a little sore but not too much.  Talk to your doctor if you are worried about this.

Many things: Cysts, tension on supportive ligaments, infection, other masses, etc...  Symptoms need to be mapped closely.  Get as much family history as you can and follow up with pcp or ob/gyn asap.  Supportive measures such as decreased caffeine intake and support bras can sometimes be helpful.

Yes: They are very accurate in hands of a competent breast radiologist.  If there is a discrepant result between the pathologist's finding and the radiologists impression, it should likely be rebiopsied or at the very least closely followed.
