Ent: Any discharge from the ear?  Possible ear infection so seeing a health provider would be a good idea.

i get a sharp pain on the lower back and it throbs, pulsating with the heartbeat. it throbs going up the spine and i can feel my whole back up to the neck throbbing painfully. this usually happens when i'm startled, causing my heart to beat fatser.

Need to examine: It is not feasible to provide a meaningful opinion without taking additional history, physical examination and may be some tests. It would be prudent to see your doctor and discuss if you may be depressed. Counter-intuitive as it may seem, exercise may help. Start with low intensity exercise such as walking and gradually increase the intensity as tolerated.  Wish you good health!

Fluid from nose: Is it clear?  Does it taste salty?  If so, it could be a spontaneous cerebrospinal fluid leak (CSF). You should collect 1-2 cc of the fluid and get it tested.

Can't  say for sure: Coudl be TMJ.  I have  no data to indicate  arteritis. I agree more than one process can be active. The complexity here indcates your  best  benefit is to discuss this with your physician.

Depends: If this is new/sudden onset, then i'd talk to your doctor immediately!  if this has been going on for a while and has progressed, there are things that can be done.  Sometimes, neck issues can trigger headaches.  Sometimes they are independent.  Mri studies may be helpful.
