why is hyperparathyroid disease so hard to diagnose?

No symptoms.: Most time asymptomatic. Usually found  b/o calcium levels are high several times then we order the pthintact ( intact parathyroid hormone high).The problem is that high calcium may be caused by cancers and many other entities like multiple myeloma, familiar hypercalcemia, secondary hyperparathyroidism.  We also order 24 urine collection of calcium to assess the excretion.

Hyperpara: You can drink a lot of water to try to prevent kidney stones.  You can take drugs like Fosamax (alendronate) to try to prevent bone loss.  There is a drug called sensipar which lowers calcium levels but does not prevent the bone loss.  In short, if you are 41 and have primary hyperparathyroidism, you should probably have surgery.  You should be seen by an endocrinologist who can refer you to a good surgeon.

Phosphate control: Parathyroid levels can be kept in check naturally by maintaining a low level of phosphate intake. Adequate vitamin d supplementation in the form of cholecalciferol is also helpful.

Sometimes a cure for secondary hyperpara is not possible...

"Classical" symptoms: would include bone pain, osteoporotic fracture, kidney stones, and stomach or GI issues.  However, many people have a broad range of symptoms and they may be surprisingly unrelated to the degree of high calcium or parathyroid hormone levels.  If untreated, ultimately vascular disease, heart rhythm disturbances and stroke and kidney failure can all result.  If you meant "Signs", these are few.

how do you diagnose hyperparathyroidism?

Medical tests: Parathyroid hormone (PTH) regulates how much calcium gets into the blood from bones to make cellular processes work correctly.  First blood calcium has to be significantly high (>11 m/dl). Then PTH can be directly measured in blood. If high this is usually caused by a benign (non cancerous) adenoma of a gland in the neck which can be identified by a radioactive scan and then removed by surgery.
