Shoulder pain: Sounds like you have bursitis in your shoulder. It helps to alternate holding positions with your baby.  If you are breastfeeding there are several positions that can take some pressure off your shoulder including the football hold.  More about breastfeeding positions:

.: Hormonal changes cause enlargement of ducts in the  breast causing pain and engorgement. Otc pain relief for pre menstural pain might help.

small (smaller than pea) smooth, round breast lump upper left breast. tender to touch. had appointment with nurse who said she wasnt too worried and that i should wait and see if it goes away after 3 weeks. im 25 and scared it is breast cancer?

: Breast Cancer is very rare in women under 30 years of age. So it is unlikely to be the case with you especially because the size the lump you have felt is too small to take it seriously. I would suggest that you watch this lump for some months and check its size once per month. Unless it grows in size to 1cm, it is not likely to be breast cancer.
Benign lumps like a fibroadenoma is more likely.

Hormones: Elevated estrogen is generally the culprit, at 48 you may actually be making more (esp if u r bleeding more) a low dose ocp would help.  Sometimes bra too tight but cancer doesn't present this way, keep up with the mammograms though.

Increased blood flow: Hormonal changes and increased blood flow both contribute to the sense of warmth in tender breasts.

Breast soreness: If the breast is reddened and warm it would suggest mastitis due to infection. Is there any sign of a bruise? Otherwise there could be congestion from blocked duct or growths. Apply warm compresses and see your md for evaluation.
