Its elective: Some of the issues associated with Down's syndrome and spina bifida require specialty hospital/surgeons to be available, so knowing the results ahead of time will allow your physician to either have you deliver at those sites or consult someone before delivery

YES: Smoking has been shown to lower test scores with as few as 6 cigarettes per day.  Cocaine and its relatives can cause the placenta to suddenly separate, putting you and the baby in danger of rapidly bleeding to death.  Anything that goes into your system, goes to your baby.  If you are using heroin, you need to switch to methadone, but you should not stop cold turkey while you are pregnant.
