Basics: The glans (top) of the penis &amp; the coverings are supplied with glands that create a lubricating material called smegma. You will need to add soap to the process to remove accumulated material that may or may not be visible or the residual will feed germs &amp; create an odor.

Smart to use condom: If all you noted was true, chances are pretty good that you have avoided an unintended pregnancy. Continue to practice safe sex, and do not (EVER) reuse a condom for any reason even if there is no discernable hole/leak.

vigorous masturbation can lead to muscle strain and trauma that can be experienced as muscle strain or pain.

I would also worry about trauma to the tubing inside of the scrotum.

But, rest is often warranted. This would mean a few days without masturbation. Depending on the intensity of the pain adding an anti-inflammatory is also advised.

Hard to know: "protected" is not a medical term so the meaning is not clear   withdrawal helps but is not wholly reliable in preventing pregnancy. Since avoiding pregnancy is your goal at present, you should find a reliable method of prevention -of which there are now many.

Probably low: Your risk of HIV infection during intercourse is higher if you are exchanging bodily fluids(ejaculate inside someone) without use of a condom.  If you are using a condom with intercourse, but pull out, remove the condom and masturbate to ejaculation outside of your partner's body, then this is very low risk of HIV as there is no contact of bodily fluids from one person to the other.
