Epididymitis can also be very painful &amp; would require antibiotic treatment. Some degree of pain relief, by gently lifting scrotum by hand, would suggest epididymitis &amp; increase pain if torsion. Symptoms could also be due to hemorrhage into testicular tumor. Thus see Dr. ASAP

Urgent exam: Although testicular aching may be caused by something benign, it is always worthy of an urgent exam to rule out torsion, or twisting of the testes, which can result in testicular loss. A moving lump is often a benign cyst, but again, you can't take a chance & must be examined to be sure. Stds can also cause testicular aching. Get examined & if you're at risk get std tested also. Testicle lump: Many types of lumps can appear in scrotum.  Epididimytis is usually painful needs to be treated with antibiotics.
