For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

Could be infection: Diabetics who get a perirectal infection can get into  serious trouble very quickly.  He needs to see a doctor today to make sure that either, a) he doesn't have an infection, or b) if he has one, it gets treated quickly.  Sometimes an urgent operation is necessary to prevent or treat worse problems like sepsis or death.

Endometriosis?: See you Family Doc and get checked out for endometriosis. Your symptoms sound severe, especially to have started in your teens! It is especially telling if you have deep-penetration sex pain. Plan on having a pelvic exam and an ultrasound for a complete evaluation. If it's not endometriosis, then those investigations will take you closer to a diagnosis and then a treatment.

You need an exam!: The most common causes for blood in stool and anal pain are hemorrhoids or anal fissure and can be treated with medication or rarely surgery. You must see a primary care doctor to confirm this. Other causes are colitis, colon polyps and colon cancer. If you don't have an obvious anal fissure or hemorrhoids, then you must see a GI specialist to make sure you don't have any of these conditions.

Spasms: These most likely are spasms of the levator muscles around the anal area and are quite painful although transient. Sitting on the toilet tends to relieve them after some minutes. These spasms are sudden and spontaneous and if they occur frequently try taking magnesium tablets. A proctologist May offer more insight and advice about this problem which is not rare.
