Elevation of bilirub: Isolated in a young man can be due to Gilberts disease which is a benign congenital condition with defect in the glucorindation of bilirubin. However the etiology of the fatty liver is unclear. I guess a GI specialist will help since your BMI is normal

Sex: It might sound strange but you should make an appointment to see a Psychologist. If it is as I suspect you will be put on track to get better over time. The psychologist may also suggest you see a primary doctor for a full physical exam.

Need further testing: You are absolutely correct, pain in your back and a generalized bad feeling after a fatty meal may mean there is a problem with your digestive system.  Given all of your symptoms it would be best to see your primary care doctor for consideration of more blood work and possibly further testing.

Fatty liver common: Fatty liver occurs in many americans, ; correlates with obesity, elevated cholesterol, diabetes, significant alcohol intake. Fatty liver can progress to cirrhosis, so patients with fatty liver (especially if abnormal liver tests) really need to: stop drinking, lose weight, control their sugar ; cholesterol. So  far so good with your x-ray in that tumors are not described on current films.

Follow MD recc: There are many reasons for elevated liver enzymes. These include alcohol, medications, infections, autoimmune illnesses, other lnesses such as copper or iron overload. The most common causes are heavy alcohol intake and fatty liver. I suggest talking to the specialist who did your tests to make sure that several causes were tested for. I suggest following your doctor's advice.

This is serious: I trust the dye test was a pulmonary angiogram that demonstrated a pulmonary thromboembolus, and it has also been determined that you have hypercoagulable blood. This is a major problem that will require work with your primary care physician and probably long-term medical management -- more than healthtap can do. Hope you do not smoke. The "inflammation" & liver labs need to be addressed in time.

Transaminitis?: Not sure what you mean by the "liver level"  but but liver enzymes commonly measured in the blood are called transaminases 9eg. Ast, alt) and when they are elevated it suggests there is inflammation of the liver and this can be on the basis of many possible causes including alcohol, fatty chage, hepatitis (infectious, autoimmune, vascular) or drugs. Liver lesions: Liver cysts are actually fairly common, there are other causes that are not so benign.  Often it is best to have complimentary studies such as an ultrasound and MRI to best evaluate the cause.
