Depends on injury: The prognosis for acute kidney failure depends on type of kidney injury. I would expect that with car accident, cause of kidney failure would be massive muscle damage that led to toxic effect on kidney from proteins released from muscle. Often improves over days or weeks, but unfortunately is sometimes permanent. Ask nephrologist what the expected outcome is. Hope things improve soon.

Yes it could: Did you fracture a rib? Could also be a musculoskeletal thing without a fracture. If painful with pressing on rib this supports musculoskeletal. Try stretching, exercises and and nsaids as needed.

It is common for this type of arthritis to be worse in the morning and improve throughout the day.  If your pain lasts greater than 2 weeks be sure to see your physician.

Be evaluated,need dx: It is important to know what your diagnosis is. Especially if the pain is still severe, you may have a cause that can be treated. Matching diagnosis to treatment is the most important step. However, after six years, it sounds like you have a chronic pain syndrome. Those are very difficult to manage. Best approach are integrated approaches with exercise, pt, etc, maybe judicious/proper use of meds.

Sever back pain: Severe bak pain needs the evalution of a spinal orthopedic surgeon or a neurosurgeon. If the pt was not a candidate for surgery in the beginning then epidural injections with anesthetics and cortisone by a pain management specialist or anesthesiologist is the way to go. Also physical therapy is helpful. Muscle relaxants and narcotics are good for a short period of time.
