Smegma or yeast: Smegma, which is a combiation of dead exfoliated cells from inner foreski 7 glans + oily secretion + moisture can irritate both glans & covering foreskin. This needs to cleaned on a daily or twice daily basis. Any tightness of foreskin will increase discomfort & itchiness. Yeast on head of penis& under foreskin can be very itchy. This will respond to anti-fungal cream or powder. Good luck.

Stuff can collect in there, called "smegma"(after that famous L.A. 70's experimental band...).And smegma should be cleaned away every day if it's there In your case,it sounds like it got infected, probably by a skin"fungus"(named after that famous Philly jam band).So wash with soap and water and apply Clotrimazole cream (get at 99cent store). No worky? See doc-y

Infection: The foreskin can get infected. Try over the counter triple antibiotic and or antifungal. Get checked if not resolving. Keep the area clean and dry as possible.

Vitiligo or fungus: White spots can be due to a fungal infection or a condition called vitiligo. This is due to loss of cells or loss of mechanisms that create the dark pigment. Please make an appointment with your doctor to make a diagnosis.

Tinea: Sounds like a fungal infection. You tried Fluconazole tablets orally? Try otc Clotrimazole twice daily for 10 days and attempt to keep area very dry.

HSV?: Clear blisters on the penis suggest hsv infection (genital herpes).  Have you had sexual activity that may have transmitted the virus?  The "white stuff" under the foreskin may just represent residual semen, so the recommendation would be improved hygiene especially after masturbation.
