? Dactylitis: swollen finger and toes could indicate dactylitis is especially if it looks like a sausage.  It would be nice if you send a picture.  Other considerations include serious infections and inflammatory diseases like rheumatoid and reactive arthritis.  These conditions can sometimes cause rapid joint destruction and need to be evaluated urgently.

See Hand Specialist: Impossible to tell with limited history but first thing that comes to mind is infectious process from either a bite (insects) or cut.  The timing matters, is this a chronic problem or did it happen recently?   Painful lesions on the hand usually indicate a herpetic (viral) whitlow.  So to really tell you how to fix it, you need to have an expert get a good history and physical exam.

Itch: Finger nails, crevices and cuticle itching would make me consider a fungal infection.  However, some type of dry skin process can cause this.  Anxiety and nerve issues can sometime present this way.

Thumb itching: Your thumb being swollen and itching sounds like a contact dermatitis to something you got on it. Try 1% Hydrocortisone but this may not be strong enought. If this begins to spread then you need to see your doctor.
