Also, the method of doing it in the lab, where an embryo is started by in-vitro fertilization (IVF), gets women pregnant without any sort of sexual activity.

Double mastectomy: Bra , containing  two external silicone prosthesis  if opted not to under go reconstruction., usually can get it with rx from your doctor , most of the time will be covered by insurance

Yes: but may not be most attractive
There are cosmetic reconstructions.
Frankly few men or women should be topless!

It depends : On the size of your native breasts.  Large breasts are obviously more difficult to achieve with reconstructive surgery than smaller breasts.  They are not normal, but should be indistinguishable from the real thing if you keep your clothes on.
