You probably: Have a fissure, as internal hemorrhoids don't  hurt unless prolapsed to the outside. Your constipation history goes along more likely with a fissure. You should ask your doctor to refer you to a colon and rectal surgeon.

He should talk: with his surgeon and determine what is causing his hemorrhoid flare. Usually his surgery should not be a cause of hemorrhoid flare unless he has been constipated or having frequent diarrhea due to his surgery. Again his surgeon should be notified and evaluate your husband

No: He probably had the hemorrhoids before, but without symptoms.  He probably was taking pain killer that will make him constipated, which can make the hemorrhoids to enlarged and cause pain with occasional bleeding.  He should contact his surgeon and be evaluated.

Hemorrhoids: One thing that is important is not to make your hemorrhoids worse. When you have a bowel movement be gentle when you clean yourself off.  You can use pre-moistened  toweletts to dab your anus. Try not to wipe/rub.  Pat the anus dry after cleaning.  Avoid using harsh soaps or ones that contain dyes or aromas.  Cotton underwear can be helpful as it absorbs moisture well in the anal area.
