Sciatic : Sciatic nerve injury is a known complication of shots in the butt (doctors, who like to use big words, call this a "gluteal intramuscular injection").  The sciatic nerve is formed by branches (called nerve roots) that come off the lumbar and sacral segments of the spinal cord.  These nerve roots rearrange themselves (in something called the lumbosacral plexus), and combine to form the sciatic nerve.  The sciatic nerve, which is the largest nerve in the human body, leaves the back of the pelvis by passing over a bony notch and under the piriformis muscle (about a third of the way over from your tailbone to your hip bone).  This roughly corresponds to the "meatiest" area of the butt, making it an erroneously attractive target for the needle.

The severity of sciatic nerve injury after injection varies with the injection site and with the substance that was injected (penicillin, diazepam and Chlorpromazine may be the worst culprits).  Severity ranges from mild, temporary sensory disturbance to severe sensory disturbance and paralysis.

Several days: Epidural steroid injections have used since the early 1950s to treat lower back & radicular pain. It is generally accepted that they provide short term relief of symptoms in at least 50% of cases, there is still no good studies to substantiate long term effect. There may several days of increased pain initially because of increase of pressure from the injected fluid or due to local irritation.

A few options of Rx: For a really bad case, surgery is a good idea.  When surgery is not the best option, pain from nerve irritation can also be remedied by physical therapy, traction, weight loss for a lumbar spine case, and sometimes medicine, like gabapentin or amitriptyline.

i am in a lot of pain from a pinched sciatic nerve caused by piriformis syndrome.  have had a steroid injection already not much help. what can i do?

EMG,PT, botox, surg: Make sure you had quality EMG ruling out lumbosacral radiculopathy. Then physical therapy involving strengthening of the pelvic and hip region and stretching of the piriformis. Besides Ultrasound-guided glucocorticoid injections, botulin toxin injections have been used. If all of the above fail, a surgery (piriformis tenotomy) can be pursued if symptoms are that persistent and debilitating.

Small: Not that these cannot occur, but considering the number of these types of injections that are done, they are rare - especially done " by the books" or with the proper technique which is by giving it in the upper outer quadrant of the buttocks, away from the sciatic notch where the nerve is..
