Not necessarily.: Smoking and lung cancer is not a sure think, it is a risk.  A better statement would be, if one smokes, the risk of getting lung cancer increases.

Yes, but...: I am very glad you stopped. If you smoked more than 100 total, you have increased your risk slightly above baseline. There's nothing more you can do about this, and your risk is still very low.

Possible: But not likely. Would still suggest that you bring it up at next visit with your PCP. Make sure you are able to characterize your pain and how it relates to different circumstances. Not too late to cut down or quit smoking.

Unlikely: If she is only 15 years old it would be very unlikely to have lung cancer at this age. However, this is a great time to stop smoking completely. Otherwise the risk of lung cancer will be higher and higher with every cigarette that she smokes. So please encourage her to stop. This is the perfect age to do it. Possibly: Cigarette smoking increases the risk of lung cancer. There is no "safe" amount. Life is full of choices, and if you smoke you inhale toxins which can harm you in many different ways. According to the CDC:

"People who smoke cigarettes are 15 to 30 times more likely to get lung cancer or die from lung cancer than people who do not smoke."

No: If you do not use tobacco again, your risk of getting cancer will not increase above the baseline risk.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

Yes.: If you smoked, you should definitely get follow-up to be sure you don't have lung cancer.  This is really important if you still smoke now.  If you quit, your risk of lung cancer goes down with time, to the point of being similar to non-smoker risk.  But you should get follow up with imaging studies of the lung, and other lung tests.
