Urologist: Fluctuations of penis size is not unusual. You may be secreting prostate fluids rather than sperm during bowel movements when you bear down. The prostate is located adjacent to the rectum. This secretion may signify some prostatic congestion. I recommend a prostate examination by a urologist to be on the safe side.

around 15-30mins after i ejaculate,i feel some discomfort in perineum area and am not able to sleep. after pooping, i could sleep. what could this be?

Discomfort: That post ejaculation discomfort could be from prostatic congestion or subsequent spasm or soreness in the levator muscles around the rectum. As long as you're not experiencing this symptom regularly or on other occasions I doubt there is any chronic pathology to be concerned about. Consult a urologist if you still have concerns,however.

Urologist: These are somewhat complex symptoms. It may be mucus rather than semen which accompanies your stools. I suspect some prostate congestion and/or inflammation or an element of epididymitis. For sperm to be expelled through the rectum, fistula from the genital tract to the rectum has to be present which would be quite unusual. Consult a urologist for evaluation and advice.

Why do it: Urine and semen are normally sterile and the stool is loaded with bacteria which may not harm you. You shouldn't contract any diseases from those activities but they seem too grotesque to consider doing them.
