None: Unless there is another problem like itching or burning, there is nothing in the line of treatment available. There may be good cosmetic products you might find.
If you really want my "truth", i'd offer an opinion that you talk to a therapist that can help you with your self esteem and self image. Where you live may be part of the issue. I hope you find other ways to value yourself. You'll like it.

See a dermatologist: No resolution to this until you are seen by a skin specialist. There are many possible approaches. I just saw a man who was scrubbing his knees, thinking that the dark color was dirt. It was not dirt. It was inflammatory pigmentation caused by prolonged and violent friction.

See dermo: Begin with a diagnosis. Acanthosis nigricans? Post-inflammatory? Genetic? Subject to repeated pressure or friction? Fade cream may help a little.

Pressure: Pressure spots will develop thick skin and dark spots.Less pressure and skin lotion will help.

: You are right; there could be skin changes in response to hormones in the body from fat cells. I think it may be what they call Acanthosis Nigricans. However there are other hormones that may signify another disease from the adrenal glands. You could see a dermatologist to see if they could remove it, but I think removing the stimulation by weight loss would change things.

Psoriasis: Rough, scaly and itchy skin at the elbows and knees is very characteristic of psoriasis. She should be seen by a dermatologist as this is a life-long problem.

Hyperpigmentation: All humans who get wounds find that the color and texture of the new skin can be different than they are used to.  Over time as the new or healed skin matures it's color often begins to match the old skin naturally.  This does not sound like a disease or condition to worry about.
