Assuming: your diagnosis is correct, topical over-the-counter anti-fungal creams work very well, but since fungal spores live for 30 days, you must keep using it for 30 days, even if it clears up in only a day or two.  If it doesn't clear, it's probably not a fungus.

Topical creams.: The best way (in my opinion) is to use a cream that kills the fungus, instead of using one that just slows down the replication.  Lotrimin ultra, or Lamisil (terbinafine) creams are easy to find and work the vast majority of the time.

See a doctor: It might be a fungus and might need an antifungal medication or it might be psoriasis or something else.  A doctor should see it.

Third best treatment: Selenium sulfide found in selsun blue kills the fungi (malassezia furfur) that causes tinea versiclolor. Applying the shampoo, letting it dry on for 10 minutes,  washing it off, do this daily for 7-10 days may do the trick. Or .... Ask your doctor for a single dose of Fluconazole or ketoconazole and watch your skin clear up.

See a podiatrist: This sounds like an issue beyond self-help measures such as "bathroom surgery'.  The thickened skin may be callusing over a bone spur, which might need to be addressed.

See a dermatologist: Probably tinea versicolor. You can try a zinc pyrethrium or selenium sulfide shampoo which you apply to affected areas, then shower off in a few hours.

Evaluation: Sounds like you have a fungal infection. Would be good idea to be evaluated as there is medication available for all of these conditions. A physician may also need to perform a biopsy or culture as well.
