It depends: Many disorders fluctuate spontaneously over time; there may be symptom-free periods (even for years). Some improve partially - e.g., some  with adhd lose the hyperactivity, but retain concentration problems, as they age. Some personality disorders may improve over time. Some disorders stay more or less the same, and some get worse over time. Treatment, of course, greatly influences illness course.

See your doctor: The best way is to see a Psychiatrist for a comprehensive Psychiatric evaluation.  They are the best trained clinicians for diagnosing these disorders.  Your family physician or internist can do a good job if well trained and they take the time.  Most of the time, making a reliable psychiatric diagnosis requires taking the time to obtain a full history, which is time consuming.

Variety of reasons: There are a lot of different reasons. Some are literally paranoid and would not take any tablets or pills thinking that ultimately they will poison them. There are some that are implementing cultural beliefs and honoring the worldviews of their relatives. Then there are some that have such high anxiety and deem it risky that are not paranoid. And some have a healthy desire to give therapy a chance.
