Yes retest: A test about 2 weeks after treatment is always recommended to make sure the chlamydia is gone.  Also any sexual partners you have had recently should be tested/treated.  Condoms are recommended to help prevent sexually transmitted diseases, even with oral sex.

5 days to 1 week: U must go for a reculture to make sure it is gone as sometimes it can be resistant and you will need another course of a different antibiotic  and make sure your partner got treated whether his culture is + or not!

Probably: Although the way you took the Azithromycin is not the recommended treatment for chlamydia, it is probably adequate. You need to get rechecked to document a cure. The partner also needs treatment, you should not have sex with him until he is.

7 days: I assume you are referring to genital chlamydia infection as there are other types.  Azithfromycin 1 gram given in 1 dose is the recommended treatment in most guidelines.  Doxycycline 100mgs. Twice a day for 7 days would be an alternative.  Do not use doxycyline if you might be pregnant.

Possibly: The best way to ensure that the chlamydia infection has been treated adequately is for your and your partner to get re-tested.  Abstinence or use of barrier protection (condoms) should also be used until the chlamydia culture returns as negative to prevent re-infection.
