Depends: Infertility is not usually so black and white. Severe bilateral chronic epididymitis will certainly increase your risk for infertility above the usual 15% risk. How much will vary widely. It is possible it could obstruct the tubes that allow the sperm to leave the body and can also affect testicular functions like sperm and hormone production. It is important that this be treated now. Unlikely...: Epididymitis is rarely acquired from oral sex. It could happen due to gonorrhea, but rarely. What are your symptoms? Pain in both testicles is almost never epididymitis, which almost always is one sided, with marked swelling and tenderness. And non-improvement on antibiotics is evidence against infection of any kind. Google "chronic pelvic pain syndrome" and see if it matches your symptoms.

Epididymitis: This is a peculiar situation. At your age, epididymitis usually is chlamydia or gonorrhea, or less commonly a non-STD urinary tract infection. Abscess is possible. Other possibilities include side effects of certain drugs. If gonorrhea, chlamydia or UTI, maybe you have not yet had the right antibiotic.
