Not how it works: Your ovaries are suppressed on pills so you don't ovulate. The ovaries are the same throughout the month. The risk of pregnancy from missing pills is the same on any day when you are on the pill. From your ovaries perspective there is not week 1, 2 or 3. There is only on the pill and off the pill.

Might be: The pill works by fooling your body into thinking it is already pregnant (hormones) , if the body detects a decline in the hormones (missed pills) you may ovulate. Doubling up may not prevent it. Precum has enough sperm to get you pregnant, but less than a full ejaculate.It is best to add a form of BC if you are forgetful.

Unlikely: It is unlikely if you take your birth control as you should.  However it is not impossible and it would decrease the likelihood even more if you used condoms.

Depends: The pill works by providing your body with a relatively constant level of hormone that tricks it into thinking you are already pregnant, so you do not release eggs.If the level falls far enough, egg release may be triggered.If you take the pill within several hours of a set time it will probably work fine.If you forget/skip or otherwise delay too much, it may happen.

Perfect vs typical: When birth control is used ideally it has a very good success rate - less than 1% of females will get pregnant.  When it is used typically it has a measurable failure rate of about 8%. Some women really can't remember to take meds, or refill on time, etc....
