Rash: Could be a rash for many reasons. This would require a doctor's visit to look at the rash and obtain more information to help determine the cause. Could be a reaction to the fabric you wear, or what you wash your clothes with, or something you are eating, etc.

Lots of ways: Back pimples are a turn-off for some women and if this is the case, you want to get treated.   I have had a great deal of success with topical Clindamycin for patients; my own acne was severe and i spent many years on tetracycline with good results.  Failing that, a dermatologist is needed to prescribe accutane. There's no reason that in the 21st century your acne can't be controlled.

Rough skin: Try using something like Aveeno for your bath water to relieve your rough skin . For acne of your shoulders you can try Retin-A ointment before bedtime

See dermatologist: Dermatologists help with skin antibacterial soaps and topical management of pimples in those areas.  Poor drainage of skin oils often can plug the drainage tubes (to surface), coupled with bacteria in the area often lead to pimples.  Do not squeeze or pick at the areas, as it may spread infection and increase scar potentials.

Manage your acne: You're almost an adult. If topical benzoyl peroxide and/or retinoic acid haven't cleared you in two weeks, your physician will need to add prescription-strength Rx. Keep doing this until your acne remits on its own. There is nothing you can do to speed its self-curing. Dark spots can be bleached but I'd advise you not to try; depressed scars need specialist intervention.

Rash: Since the rash is limited to your back and if it is on both sides of the spine, it could be a type of heat rash. A viral rash like shingles stays on one side only. Have a physician examine you if the rash persists or spreads.

Get a biopsy: This problem could be from something more simple to something related to an autoimmune related disease.  I would recommend a visit to your dermatologist 
for evaluation and treatment.
