Need to biopsy: There is no definite way to know whether it is a cancer for a benign thyroid nodule without a biopsy. You may proceed with a fine needle aspiration biopsy which is a simple needle insertion which is quick and is only a little pain however many times it will require an open biopsy for definitive diagnosis good luck!

if i don't have graves' disease or hot nodules on my thyroid, yet it's producing 2x what is normal, what is the cause?

Thyroid: Depending on what lab is actually 2x as normal will help point to the root of the problem, however, i believe in treating the patient and not the labs.  So if your signs and symptoms point to dysfunction in thyroid supplementation with iodine and l-tyrosine will help support and normalize your thyroid numbers.  These 2 are building blocks for the hormone which we normally are deficient in.

benign nodules on thyroid.  my tsh is .45, t4 -7.49, t-uptk 39.9, and t7 is 7.5. sono tech saw hot nodules-  exhibit signs of hyper--is this possible?

Possible but: not likely. Your TSH is low enough for a diagnosis of hyperthyroidism.

Stay conservative.: Generally would go with more conservative approach of lobectomy.  Unless there is a compelling reason to remove the other side leave it alone.

Total thyroidectomy doubles your risk of injury to  recurrent laryngeal nerve and your parathyroid glands.

can a hot or cold nodule be determined after it is surgically removed?

Pathology will tell: Whether a nodule is hot or cold has to do with how much iodine it takes up on a thyroid scan. Once this nodule is removed, pathology will be performed on it to see if it is a cancerous growth or not. The hot or cold nodule only refers to how the uptake is on the scan.

: They can be either way, sometimes mixed based on uptake. It is the test that determines based on their behavior.

Images: "hot" or "cold" is a term related to the appearance on nuclear medicine imaging.  A "hot" nodule is one that is too active, and secretes hormone, but benign.  A cold nodule takes up less tracer.  About 20% of these are malignant, and so they are biopsied.  Cysts will also show up cold as they are fluid rather than tissue, so this study should be interpreted along side an ultrasound.

Adenoma: The hot or cold indicates the nodule is excessively active or hypoactive.  If nodule is same color as gland, the nodule is typically an adenoma.

Accepted Answer

Not likely: "hot" nodules are rarely cancer and most often represent benign follicular adenomas.  These are also known as "autonomous" nodules because they make thyroid hormone but don't respond to the body's normal signals telling them when to stop making the hormone.  The vast majority of thyroid cancer nodules are "cold" on a thyroid scan.
