Not clear: The question is not entirely clear. A high MCV and MCH anemia is not usually due to iron defiency alone. Therefore, other factors may be leading to anemia, and iron will not treat the other cause(s). As to safety, iron infusions are usually safe, but rarely there are severe reactions. Therefore, iron infusions should never be considered until other types of treatments have not worked.

would liver problems show on a cbc? i know lft/cmp is the standard just wondering if high mch and high mcv could be liver issues or is it another prob

Poor screen: High MCV is concerning specifically for heavy alcohol use, but a savvy physician will check for B12, folate, (folic acid) hypothyroidism, and hemolytic anemia (young red cells are big). If the metabolic profile and physical exam don't indicate liver troubles, probably this sturdiest of organs is okay. Remember that a few percent of healthy folks fall a bit outside of reference ranges.

Thalassemia minor?: With normal packed cell volume, high red cell count, low MCV, I'd consider you may have thalassemia minor. This is good to know because if you marry someone else who has this, the children may have the very serious thalassemia major. Talk with your physician about this and what he/she thinks is actually going on.

You have anemia: You have some form of anemia in all likelihood. Your red cell count is may be lower than normal and that could be due to a number of factors. Your labs suggest that there is at least an iron deficiency but there could also be other problems. The high RDW tells us that your bone marrow is working to produce more cells but their quality may not be high if you are deficient in iron or zinc.

Polycythemia.: High HGB levels for 3 kinds of reasons.  First, a blood disease where too much blood is made.  Second, the blood is being made due to an increase in epo, the hormone that controls production.  Can be from lack of oxygen (lung disease, living at high altitude, abnormal hemoglobin, carbon monoxide/smoking poisoning hemoglobin, heart disease, or rarely tumors.  Dehydration can do this too.

Need more info: How high?  A high MCV can be seen in some vitamin deficiencies, drugs, and alcoholism. You are stating "the flags" on your blood work. Need to know the real numbers and why the tests were ordered. Cannot interpret in a vacuum.
