High blood pressure: Normally one doesn't hear their own heartbeat. However sound travels in the direction of (blood) flow &amp; the only way it gets amplified is if there is more. That happens frequently (but not most of the time) with high blood pressure, or hypertension (also known as the silent killer, but perhaps u r lucky it's not silent in your case). See a doctor, get your blood pressure checked during an episode.

Premature beats, PVC: You could be having premature beats, mostly pvcs or pacs. From heart the blood runs to the brain through the neck and many people having pvcs they might feel palpitations in their necks and the ears.
Might need to wear a holter monitor to assess it further.

Conductive?: The carotid artery and jugular vein travel in close proximity to your inner ear.  When the conduction mechanism bringing sound from outside to your inner ear is impaired, the internal sounds, such as your heartbeat, gets amplified. Thus, you hear your heartbeat.

Not sure: I am not sure why looking down makes you aware of pulsation in your ear.  But i can tell you what you are experiencing.  The carotid artery, the large artery in the neck that supplies blood to the brain is very close to the cochlea, the hearing portion of the inner ear.  The inner ear senses these pulsations.  I am surprised that more people are not aware of this.  Called pulsatile tinnitus.

Normal: When you move in different positions, you heart beat becomes more or less audible depending on your position and how forceful the blood is moving through your body.

Turbulent flow: More than likely you are hearing your external carotid artery.  Flow in this artery often causesa roaring in the ear on the affected side.  On ocassion, this artery may have to be ligated to stop this if unbearable.  See an ENT or cv surgeion if not your pcp and get evaluated.  You may need hearing eval as well.

Yes!: "tinnitus" and "palpitations" can be caused by stress but if significant and persistent see your doctor to make sure

: If the sensation is a steady beat you're experiencing pulsatile tinnitus. It's usually not a serious issue but sometimes it's associated with high blood pressure or increased pressure within the brain. If it doesn't resolve after a prolonged and/or heavy period, I recommend having an evaluation&amp; being advised by an ENT specialist &amp; having your blood pressure checked. Also get checked for anemia.

Lying on the: Ear, many people can hear their pulse.This is the most common cause of this complaint.
