More information: The possibilities are numerous because your symptoms are nonspecific.  That said, common things to consider include stomach ulcer, high blood pressure, viral illness, viral gastroenteritis.  Also must consider an intracranial process such as a tumor especially if headaches are new.

Different: If this is new or different than your typical migraine, then time to get checked out.  I typically advise patients to call the office if the migraine does beyond 48 hours.  There are a number of headache rescue therapies available.

Medical Evaluation: Blood pressure needs to be checked. Significantly elevated blood pressure can sometimes be a cause for this dyad of symptoms.  Eval will help to determine any other possible causes.

Now: She needs to be seen by her primary care doctor and have her BP checked and baseline labs, such as creatinine, sodium, potassium, thyroid.  Also see an ENT doctor to evaluate the nose for bleeding lesions.  Big concern is high blood pressure leading to increased pressure in the nasal passages causing bleeding.  Hope this helps.
