Probabaly but: You should be evaluated by your gynecologist just to make sure. Your diet, stress, poor sleep etc. May be contributing to what's going on but best to make sure that's all it is, which is probably all it is.

Lose Weight: The amount of menses is diirectly related to the thickness of the lining as well as the size of the uterus.  Estrogen is what leads to thickening of the lining.  Fat binds estrogen so higher levels are needed in the blood for a women to release an egg and complete a cycle.  So weight loss in a women who is overweight lightens the menses.

Periods: I would recommend birth control pills if you are able to take them.  An ultrasound may show reasons why and offer additional treatments.

Perimenopause: Is likely, but you should go see your doctor to be sure.  The periods every 5 month is fine (perimenopause as you say), but the bleeding twice a month could be a concern.

Peri-menopause: It is likely that you are entering the peri-menopause.  The years prior to menopause women will frequently complain of longer cycles, irregular bleeding, missed periods.  See your gyn to confirm that this is what is happening to you and to rule out other conditions that can cause heavy bleeding and mid-cycle bleeding.  This is very treatable.

Yes, get med eval: Now ! normal menstrual periods do not last longer than 7 days. Possible causes / risks for excessively long periods include but are not limited to: uterine fibroids, polyps, adenomyosis , endometrial cancer, type i diabetes mellitus, bleeding disorders (l.E., von  willebrand dz), clotting problems, infection, ovarian cyst, endometriosis ; hashimoto thyroiditis.

Irregular periods are one of the most common problems Obgyns treat.: If this is one time problem it usually resolves on its own. If this is a recurring problem you should be evaluated. Infections &  hormonal changes can cause spotting. If there is a concern for pregnancy then do  a home pregnancy test .  there are many options available to help regulate the menstrual cycle. Schedule a visit.

Normal: any variation in flow can be very normal, notify your doctor if drastic changes, pain, heavy bleeding.  You may need to check a pregnancy test thyroid hormones, other hormone tests or blood count to make sure you are not anemic
