Keep it dry: Retracting the foreskin before urinating and during bathing might help as long as the area is dried off well.  Chronic moisture exposure can leave the area irritated, leading to the symptoms you have.

Need to examine: It is not feasible to provide a meaningful opinion without taking additional history, physical examination and may be culture from the affected area. You should consult a urologist and if need be consider circumcision.
Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

Diabetic: I think self-treating diabetis with non-prescribed Insulin is asking for trouble. You need to see a physician. Cuts in your foreskin need to be treated and you may need a circumcision.

Consult urologist: You most likely need 2B circumcised, however there is an outside chance that you might respond to course of prescription strength steroid cream, unless foreskin is scarred from previous tears.
