No magic age: I generally recommend beginning foods at 4 months.  Some doctors recommend six months or later.  Some parents begin earlier than 4 months without ill effects, and some hold off  and breast feed for an entire year  (which is pretty remarkable).    You should ask your pediatrician his/her opinion.

Baby cereal : When your baby is 4-6 months old it's usually time to start trying solid foods.  For most babies the first food is rice creal or infant oatmeal mixed with breastmilk or formula.  These baby cereals provide a great source of iron for your growing infant.  Other early options are banana, avacado or cooked potatoes and sweet potatoes mashed and thinned with a little breast milk.

Guidelines: When an infant has enough upper body control to indicate hunger and satiety, is taking > 32 oz. Of formula/day or nursing > 8-10 times/day,  & is 4-6 months of age. Use single-ingredient baby foods, don't start with the sweet stuff. Blend " people food ", especially if you can hold off till 6 months. A beaba works well, but is expensive. If baby spits food back  out, the meal is  over.

Not necessary early: During the 50's & 60's when moms made their own cow milk based formula, it was important to feed solids to make up for the deficiencies of cow milk.Today the breast or commercial formula fed babies need no solids the first year. We have a  need to teach them the different skills needed to feed solids & most are ready @ 6m when they start sitting with assistance. This is a good time to start.

At 4 to 6 months old: It's best to wait until your baby isn't filled up enough with breast milk or formula at each feeding. When it's time, start with a brown rice baby cereal (more nutrients and fiber than white flour), mix it with a little water, breast milk, or formula, and give it by spoon, not in the bottle. Gradually increase the number and volume of solids as your baby's doctor recommends.

Acid reflux vs other: This could be acid reflux which is common in infants.  Reflux usually causes vomiting through the mouth or nose after feedings, with abrupt position change, or when pressure is placed on the abdomen. The severity of reflux varies.  Projectile vomiting, green vomit, weight loss and lethargy are concerning for more serious GI issues and should be evaluated immediately.

Breast milk/formula: are all that is needed until infants are at least 4-6 months old.   Early introduction of solids has been associated with food allergies and over feeding and obesity. Breast feeding protects against diarrhea illness Also, some high fiber foods may cause diarrhea, gas, cramps.   After 6 months, milk may not provide enough iron for blood and muscle growth, so foods that contain iron are recommended.

Soft: Soft foods such as fruits and vegetables , cheese and yogart, pasta breads etc.
