Scalp lesion: This may be an infection e.g. A staph infection.  You need medical attention.  See your doctor for examination. Meanwhile use a medication neosporin oint  on area three times per day until you can be seen, as soon as possible.  Go to walk in clinic if you cannot see your doctor.

Probably benign: He may have rubbed it during the night. Watch it over the next few days and it should resolve by itself. If not, see your pediatritian.

Typical location: of a cold sore is closer to your lip than your chin. A picture would be worth a thousand words.

Doubt wart: I am not sure where this breakout is located but if on or around outside of lip then is probably herpes labialis or fever blister. Often get recurrences of this in same spot over time. Have you had before. See your doctor during acute phase for diagnosis and treatment.

Many possibilities: Proper evaluation of a blistery rash on a 1 month old will require an exam. There is a possibility that this could be an infection. Please set up a visit to evaluate the issue.

i use to shave my face once a week but whenever the ocassion arises and i have to get shave after 2 days. small blisters and rash occurs on neck around trachea and around my chin. what is the cause n medically what this condition called? thanks

Rash: most likely folliculitis from shaving.  Could also be tinea barbae which is a fungus.  Topical medcations can help this  A picture and a virtual consult would be helpful
