Talk to your doctor about forms of physical therapy and conditioning and discuss the exact nature and severity of your spine and what options you have. Get a consult with a physiatrist and keep a positive proactive attitude

Knot on neck: As you state that there is no pain, likely you have a cyst, or lipoma. These do not have to be removed unless they become a worry to you or get large enough that you wish them to be removed.

Normal anatomy: This is a very frequent question and for most patients, the "knot" at the base of the neck is normal anatomy - the spinous processes of C7 and t1 are larger than the adjacent spinous processes, which gives the illusion of a knot.  Patients never notice that until they start having pain and reach back there.  Your smaller knot may very well be a spinous process also.  See an md if symptoms persist.

Fatty neck tissue: Find the  cause and eliminate it (if possible). Hormone imbalances can causes this--classically this has been a sign of hyper-cortisolism. Drugs for HIV cause this--but usually you will need to take these drugs. Excessive fatty tissue and high bmi promote this, and you treat that with plenty of exercise and much less food.

Need to examine: It is not feasible to provide a meaningful opinion without taking additional history, physical examination and may be some tests. It would be prudent to see your doctor and discuss if you may be depressed. Counter-intuitive as it may seem, exercise may help. Start with low intensity exercise such as walking and gradually increase the intensity at tolerated.
Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

Neck Lump: Have it evaluated by your primary care physician.  Ther are numerous causes for neck masses or lumps.  Some are more serious than others.  Seek an evaluation.

Neck: This could be normal as one of your neck vertebrae is larger at the base of your neck.
