i fell hard three days ago, landed on my hands/knees.  soon after  the right side of my chest and rib cage became very tender. pain has diminished some. still hurts to lay on my back or to press on chest. did not fall on torso just hands/knees. ???

The impact of fall: Although you fell on your hands and knees, the fall impacted your whole body, hands/knees were in the forefront but the wave was transmitted to your core as well. Kindly rest, use nsaids/acetaminophen, unless contraindicated, if no improvement see your doctor, wish you wellness

Xrays: For an isolated fracture the only way to be sure is to get x-rays and maybe a cat scan.  If you have multiple fractures this can cause a segment of your chest to be loose. This can be seen w/o x-rays.

Balanced diet: Agree with dr. Merriman. Key is complete and balanced diet.  Supplemental vitamins and minerals as needed under the guidance of your family physician.  That said, the most critical thing in rib bruise and/or minor fracture is to keep active, deep breath and simple stretching exercises.  Do not tape or wrap or restrict your chest.

Yes: Yes, if you note shortness of breath, severe chest pain, productive cough (especially if coughing up anything that has any blood streaks in it) or if you feel a "crunchiness" to the skin overlying the possible area of rib fracture.  These signs would suggest complications.
Otherwise, not necessary to go to the hospital since there is very little that can be done for a fractured or bruised rib.

having some internal pains from adderall (dextroamphetamine and racemic amphetamine). mostly upper torso and full back. shooting pains, nothing too bad. tender middle chest. what could this be?

Many possibilities: Good idea to run this by your doc as adderall (dextroamphetamine and racemic amphetamine) can effect bp &amp; cardiac function as well as peripheral blood flow in some cases

: It sounds like you are having a muscle spasm of the muscle in between the ribs. This was likely triggered by the same injury that caused the rib fracture. A rib fracture can take months to heal because the chest wall is constantly moving with breathing. So, because it takes so long for the rib to heal, the surrounding muscles can spasm. Avoiding strenuous activity will help.See doctor if continues

Rib pain: We have cartilage and muscle in-between our ribs that can get inflamed - even if we didn't fall or hit them.  Sometimes all it takes is a quick twist of the body or picking something up.  And often the pain starts a day or so after the event.  If it's not going away or if you have any shortness of breath or worsening symptoms, make sure you check in with your doc!

Its a good idea: to find out if you broke a rib since this will take 4-5 weeks to heal (There is no specific treatment except pain medication and ICE/HEAT) 
Hope its just a bruise since this will feel better within a week!

Further evaluation: Since you're still having other symptoms that would not be explained by a bruised rib you need to further evaluation to clarify your symptoms
